As a new patient, in order to save time during your first visit, you may complete the forms below.  You may fax, mail or bring them with you when you come for your first appointment.  If information if faxed, please send copy of driver’s license and insurance card.  However, if this is not possible, come in ten minutes early to complete our forms.

Your initial visit will consist  of a comprehensive exam that will check your gum health and an oral cancer screening.  We will take any necessary x-rays, provide oral hygiene instructions and answer any questions you may have.  If your gum health is good, we may also clean your teeth at this visit.
